For quarterly 941 does the wages reported have to include items such as health insurance and auto? Also, do most states follow federal convention (I'm curious most about NJ)? Thanks

Can't speak to NJ rules, but yes, SEHI should be included on Form 941 Line 2 wages, but should not be included on Line 5 wages. As to auto, if he is driving a company vehicle and has personal use, it is supposed to be included in wages quarterly, but many do not get it in until year end.
