RE: Q's on SS-4 for surviving spouse who I assume is to be executor (E)?

H died 7/1/09. 2008 H & W filed MFJ

Box 7a - Do I need to name E...I don't believe so since listed on Box 3.

Box 10 - Death of Spouse?

Box 11 - 7/2/09?

Box 12 - December?

Box 13 - 0 0 0

Box 16 - Other_________Estate?

Or is Boxes 11 - 16 not required since this is not a business entity?
