I believe the HSA contribution shown on a W2 and reported under code W reflects a pre-tax contribution to the HSA. As such, the contribution should not be reported on Form 8889 and as an adjustment to income on Form 1040, as that would amount to taking the same deduction twice.

Can someone please confirm my understanding?

W = employer contribution.

But employer contribution also includes anything from as sec 125 plan. Don't deduct it again when computing AGI.

There is a place on form 8889 for the ER and the EE contributions. Show the W code item as an ER contribution, not deductible by your client.
