Could a Payroll company with hold issuing an employee their W-2 due to the employer not paying them their payroll fees?

Yes; this is just like withholding delivery of a tax return until payment is received in full.

Sure. Employer is the one with the obligation to provide W-2.

And if the employer changes payroll companies mid-year, there's a good chance that the W-2's will only include the payroll of the new company and that no W-2's will be issed by the old company. I sent a full day in early December doing W-2's for a client that changed payroll service in mid-February.
