Hello everyone,

I've got a client who can only provide me with her last pay stub for the year. I know I need to prepare a form to send along with the return since there's no W-2 but I'm not sure which it is. Do any of you know?

Before completing 4852 I would ensure my client has made every reasonable effort to obtain a copy of W-2 from employer.In fact you have to respond to your efforts on question # 10 of the form.

I prefer to not work with pay stubs to create a 4852 unless its the last resort. In some instances the form itself does not contain columns for all information that has to be entered on a tax return. Examples include dependent care benefits provided by employer, all box 12 information including 401 K contributions, and taxable life insurance benefits. Also many of the payroll systems do not show taxable wages on the pay stubb but gross wages.

Recommend you be very careful when transcribing information from the pay stub to the form 4852.
