My client, a US citizen had lived abroad for several years in South America. His foreign born spouse still resides in South America. Now, my client would like to obtain an ITIN for her in order to file a joint return. Is something like this possible by mail? Must she find an acceptance agent in her country?

You can file W-7 form along with the Joint Return and apply for the ITIN with the return.

In the SSN number field, you can enter "Applied For". Please review the W-7 instructions to make sure that the required documents are sent. Your tax software might have that form also.

As mentioned above, the W-7 should be attached to the Form 1040. For the documentation, you need to have her send either her original passport (not recommended) or send a notarized or certified copy of the passport. Often the easiest approach is to get a notarized copy of the passport from a U.S. notary (available at U.S. embassies and certain consulates). Foreign notaries must comply with the requirements of the Hague Convention.
