| I have a MFJ who has a family HDHP through her employer. She is under 50 and her husband is over 50. On her W-2 for 2006, Code W is reported as $3100. Her health plan is a HDHP with a $2400 deductible, her husband is covered by this plan and does not have a plan of his own. The employer has set up two HSA accounts, one for her and one for her husband. The one for the client is funded with $2400. The one for her spouse is funded with $700 (catch-up provision). When filling out the HSA Form 8889, I am a bit confused how to report the contributions, can I report $2400 as "employer contributions" on the client's 8889 and then report $700 as "employer contribution" on the spouse's 8889 even though only the client is an employee.
I believe the entire contribution is reported as Code W since one of the rev procs states that employee contributions are treated as employer contribution for the purpose of W-2 reporting. Just wondering if anyone else has run into this. Thanks | |

| Amounts reported on the W-2 form are either employer or pretax employee contributions. Those amounts are not deductible on Form 8889. | |
