If there are any Form 5500 experts out there, I have a few questions regarding Scheudule SSA of the Form 5500. There seems to be some confusion between some colleagues regarding what should be reported on this Form.

Regarding Code A of the Form, it is clear that when a participant separates from service and is entitled to a future benefit then they must be reported on the Schedule. A colleague believes that each current participant in the plan must be reported on this Schedule, even if they haven't been separated from service and that they are reported under Code B of this form. I believe that this is incorrect. Any thoughts?

Further, I have another colleague who believes that when a previously separated participant (who was already reported as Code A) takes distributions from the Plan, then the participant must be reported on Schedule SSA as Code D each time they take a distribution. I believe that this is also incorrect. Any thoughts on this as well?
