| Anyone know where to get a template for the required taxpayer written consent form for disclosure of tax information? I just became aware of new language requirements that make my old form obsolete.

| Why do you need to disclose? If you need it for a loan product, the bank should be providing you with the appropriate form.

| I would fax the return to the client and then he can fax it to the 3rd party. Makes me feel better that way. When you see a small puddle of water, would you put ur foot into it to see how deep it goes after? hehe =) | |
