If employer has a 401(k) plan and employees contributed and employer match the contribution...if employees have elected to contribute before tax income then the contribution by the employee would show up on W2 Box 12 code D...can someone confirm that ??? PLUS do I add that contribution $$ (employee) to the Box 3 and 5, so Box 3 and 5 will be higher than Box 1 because of the contribution....thanks for your comments...

Exactly. Yes, employee 401K contributions are not subject to income tax, hence not included in box 1. They are subject to social security and medicare taxes, so are included in box 3 & 5. Yes, they do go into box 12, code D.
